Let’s get started.

1. Create your account on our patient portal and complete the initial intake form.
2. Log in to the portal and fill out the medical questionnaires and consent-to-treat forms.
3. Once submitted, our team will be notified and you’ll receive an invoice for your lab work.
4. After payment, your lab request form will appear in your portal under “Labs & Documents” (on the right-hand side).
5. Take that form to any LabCorp location for your blood draw.
6. Once we receive your results, we’ll schedule your consultation to review everything and provide a fully personalized hormone optimization plan.
